25 patients who had undergone a modified Lange repair were examined postoperatively after about four years, both clinically and by CT arthrography. Ventral redislocation was no longer seen postoperatively. As regards residual ventral instability, there was agreement between CT arthrography and the clinical findings in 20/25 patients (90%). Newly developed or residual dorsal instability could be accurately demonstrated by the presence of pathological changes of the dorsal labrum and by the observation of a wide dorsal joint recess. Mortalit�t, Morbidit�t und funktionelles Endergebnis nach "Open-book-Verletzungen" und lateralen Kompressionsverletzungen des Beckenringes

In einer retrospektiven Studie wurden die Begleitverletzungen, Behandlungsmerkmale, Morbiditat, Mortalitat und das radiologische und funktionelle Endergebnis von 222 Patienten mit einer Beckenringverletzung vom Typ B oder vom Typ C nach der Klassifikation von M.Tile eruiert. 52 Patienten wiesen eine "Open-book-Verletzung" auf (Typ B1), 48 Patienten eine laterale Kompressionsverletzung (Typ B2/B3), 122 Patienten wiesen eine C-Verletzung auf. Die Stabilisierung proximaler extraartikularer Tibiafrakturen stellt unter Verwendung der gebrauchlichen Osteosyntheseverfahren unverandert ein Problem dar. Die Wahl des Operationsverfahrens hangt unter anderem von der Situation der oftmals erheblich kompromittierten frakturumgebenden Weichteile ab. An der proximalen Tibia besteht zusatzlich das Problem von Fehlstellungen der Fraktur. Proximale Verankerung von Hüft-Endoprothesen mit poröser Oberfläche: 6-Jahres-Ergebnisse

PROBLEM The anatomically shaped pcl (ESKA) total hip replacement has a porous coating of the proximal stem and the cup to allow for bony ingrowth. The tip of the stem is polished. METHOD We report the results of 96 THR 6.2 years (5.25-6.7) after implantation. RESULTS There were no revisions during this period. One stem showed radiographic loosening at follow-up. There was stress-shielding in 9.4% in contrast to the concept of proximal fixation. Thigh-pain was found in 9.4%. One patient demonstrated disabling thigh-pain despite radiologic bony ingrowth. Surgery for fracture of the calcaneus. 5 (2-8) year follow-up of 20 cases.

20 displaced intraarticular fractures of the calcaneus were treated by open reduction and internal fixation. Results were evaluated according to a standardized protocol. With an average follow-up of 5 (2-8) years, results were excellent or good in 15 cases and fair or poor in 5 cases. An accurate radiographic study and CT scanning are essential for understanding these fractures. We feel that restoring calcaneal shape as anatomic as possible, getting an appropriate reduction of the posterior joint surface by a surgical procedure with early mobilization, and delayed weight bearing would be a good option in treatment of these fractures.

Repair of osteochondral defects in articular weightbearing areas in the rabbit's knee. The use of autologous osteochondral and meniscal grafts.

Repair of osteochondral defects in articular weightbearing areas presents its own particular problems because of the low potential of hyaline cartilage for regeneration. Our first group of experiments on the knee of the rabbit confirms that the new regenerated cartilage comes from bone marrow which degenerates before developing into true hyaline cartilage. The second group of experiments shows that autologous grafts from the non-weightbearing articular area suitable for the repair of defects in weightbearing areas. In an third group, autologous meniscal fibrocartilage was used as a graft for the repair of osteochondral defects.
